Each student is required to submit a number of acceptance forms and materials by their respective deadlines. While most forms can be submitted online, some may be completed on paper, signed, and mailed to our office. Students are encouraged to print the Student Checklist to help keep track of required steps and deadlines. Please contact us if you have any issues completing the forms.
Note:
- Families wishing to apply for financial assistance through IUHPFL should complete the Financial Assistance Application by February 1. This is optional.
- The "Statement of Medical History" form contains a section to be completed by a medical professional. We encourage you to schedule an appointment with your doctor as soon as possible to ensure the form is completed by the deadline.
